Improvement to self-service elevation report

I would like to be able to define additional selectable fields for self-service elevation (in addition to current free text reason field).

I would like to specify the fields and available values centrally for agents. For example field could have values 'Software installation', 'Software uninstallation', 'OS configuration change'

Reporting should be able to filter and group data based on these fields for easier reporting.
